Information and Consent

Obtaining and maintaining informed consent is one of the most critical, yet often time-consuming, aspects of private practice management. Consent requirements evolve with changes in professional codes, legislation, technology, and clinical practice. Poorly worded or incomplete consent can create significant risk for both clients and the practice if issues arise during therapy.

This module breaks consent and information provision into clear, manageable components tailored to different client demographics and service types, including individual therapy, couples, family therapy, group therapy, and assessments. Rather than a single exhaustive form, the module allows clinicians to provide relevant consent forms at the right stage of the client’s journey, reducing overwhelm and ensuring clients are fully informed.

Included in this module are:

  •  Information and Consent policy and procedure

Client-Facing consent forms (demographics and service types):

  •  Assessment (Adult and Child)
  •  Individual Therapy (Adult and Child)
  •  Group Therapy (Adult and Child)
  •  Couples Therapy
  •  Family Therapy
  •  Supervision
  •  Employee Assistance Programs

Additional consent forms for specific scenarios:

  •  Parent-Only Session
  •  Observing a Child at School
  •  Recording/Observing Sessions for Supervision
  •  EMDR

This approach ensures clinicians are providing consent in line with the latest APS Private Practice Management Standards and the AHPRA Code of Conduct (2025 release), while also allowing the practice to adapt to new therapies, technologies, and policy changes. By having tailored forms, clinicians can discuss consent progressively with clients, maintaining clarity and legal compliance without overwhelming them at the outset.
